SMS Terms and Conditions

1. Program Description

Freeman Family Medicine offers an SMS text messaging program to communicate with patients regarding healthcare administrative matters. By opting in to receive text messages from our practice, you agree to these SMS Terms and Conditions.

Messages may include, but are not limited to:

  • Appointment reminders, confirmations, cancellations, and scheduling updates.

  • Appointment scheduling and follow-up instructions.

  • Office hours, closures, and general practice announcements.

  • Billing notifications and payment reminders.

  • Requests to contact our office regarding administrative matters.

  • Other non-emergency communications related to your care and interactions with our practice.

  • Telemedicine Services

2. Consent to Receive Messages

By providing your mobile phone number and consenting to receive text messages, you authorize Freeman Family Medicine to send SMS messages to the number you provide.

Consent to receive SMS messages is not a condition of receiving medical care or purchasing any services from our practice. You may choose not to receive text messages and may request alternative communication methods by contacting our office.

If you provide a mobile phone number that belongs to another individual, you represent that you are authorized to provide that number for the intended communications.

3. Message Frequency

Message frequency varies depending on your appointments, interactions with our office, and communication preferences.

You may receive multiple messages in a month, particularly when appointment reminders or scheduling updates are necessary.

4. Message and Data Rates

Message and data rates may apply to text messages sent to you by our practice and to messages you send to us. Your mobile carrier's standard rates and charges may apply. Please contact your wireless provider for information about your messaging plan.

5. Opt-Out Instructions

You may opt out of receiving SMS messages at any time by replying STOP to any text message from our practice.

After you send STOP, you may receive a final confirmation message acknowledging you opt-out. You will no longer receive messages through the applicable SMS messaging program unless you subsequently provide consent again.

If you need assistance with the messaging program, contact our office at 501-327-0110.

Opting out of SMS messages does not cancel appointments, discontinue medical services, or eliminate the need to communicate with our office through another available method.

6. Privacy and Confidentiality

We respect your privacy and handle patient information in accordance with applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA), where applicable, and our Notice of Privacy Practices.

SMS messages may be visible to anyone with access to your mobile device or notifications. Standard text messaging may not be encrypted end-to-end, and we cannot guarantee the confidentiality of messages while they are transmitted or displayed on your device.

To help protect your privacy, our practice will take reasonable steps to limit the sensitive information included in SMS messages. We encourage you to avoid sending sensitive medical information, passwords, financial account details, or other confidential information by text message.

For questions about how we use and protect your information, please review our Notice of Privacy Practices or contact our office.

7. No Emergency Communication

SMS messaging is not intended for emergencies or urgent medical concerns. Text messages may not be monitored continuously, and delivery or response times are not guaranteed.

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Do not rely on text messaging to obtain emergency medical assistance or urgent clinical advice.

For medical questions that require clinical attention, please contact our office directly at 501-327-110

8. Delivery Disclaimer

We are not responsible for delayed, undelivered, or misdirected messages caused by mobile carriers, network interruptions, device settings, or other circumstances beyond our reasonable control.

You are responsible for keeping your contact information current and notifying our office if your mobile phone number changes.

9. Supported Carriers

Wireless carriers are not responsible for delayed or undelivered messages. Message availability may depend on your mobile carrier, device, and network coverage.

10. Changes to These Terms

Freeman Family Medicine reserves the right to update or modify these SMS Terms and Conditions at any time. Updates will be made available through our website or other appropriate communication channels. Continued participation in the SMS messaging program following applicable updates constitutes acceptance of the revised terms, to the extent permitted by law.
