WILLIAM H. FREEMAN, MD, FAAFP
Dr. William H. Freeman is a Board Certified Family Physician and Fellow of the American Academy of Family Physicians. He has been in practice in Conway since 1996. He earned his medical degree from the University of Arkansas for Medical Sciences in 1981 and completed his residency in Family Medicine at UAMS-AHEC Pine Bluff in 1984, where he remained on faculty for 12 years prior to moving to Conway. He completed his undergraduate degree from Hendrix College. He is on-staff at Conway Regional Medical Center and currently serves on the Primary Care Accountable Clinical Management Committee. He serves as Medical Director of the Conway Regional Wound Center. He is also Medical Director and sees nursing home and rehab patients at Heritage Living Center. He is currently accepting new patients, from adolescents to geriatrics. Dr. Freeman has followed a plant-based diet since March 12, 2017.
WILLIAM A. FREEMAN, MD, FAAFP
Dr. Alex Freeman is a Board Certified Family Physician and Fellow of the American Academy of Family Physicians. He joined Freeman Family Medicine in 2011 after completing his residency in Family Medicine at UAMS-AHEC Fort Smith. He served as Chief Resident during his final year of training. He earned his medical degree from the University of Arkansas for Medical Sciences in 2008 and completed his undergraduate work at Hendrix College. He is on-staff at Conway Regional Medical Center and currently serves on the Primary Care Accountable Clinical Management Leadership Committee and the Specialty Care Accountable Clinical Management Performance Improvement Committee. He is Medical Director at Hospice Home Care in Conway and is also on-staff at the Conway Regional Wound Center. He also currently serves on the Conway Regional Physician Hospital Organization Board of Directors. He has served on the Board of Directors at Conway Interfaith Clinic. He is currently accepting new patients, from newborns to geriatrics and everything in between. Dr. Alex has followed a plant-based diet since March 5, 2014.
AMY BURTON, APRN
Amy Burton is a Family Nurse Practitioner. She joined Freeman Family Medicine in 2007. She earned her Advanced Practice Nurse degree from the University of Arkansas for Medical Sciences and completed her undergraduate degree at Arkansas Tech. Originally from Atkins, she now lives in Conway. She works as a team with Dr. William H. Freeman and our nursing staff to provide care for patients of all ages. Her clinical interests include adult medicine with an emphasis on women’s health.
MICHELLE CHURCHILL, APRN
Michelle Churchill is a Family Nurse Practitioner. She joined Freeman Family Medicine in 2013 after working in women’s health for 11 years at St. Vincent Medical Center and The Women’s Clinic in Little Rock. She earned her Advanced Practice Nurse degree from the University of Arkansas for Medical Sciences and completed her undergraduate degree at the University of Central Arkansas. She works as a team with Dr. Alex Freeman and our nursing staff to provide care for patients of all ages and maintains a strong interest in women’s health. Michelle has followed a plant-based diet since March of 2014.
CHERYL BLAIR, LPN
Cheryl joined Freeman Family Medicine in June of 2018. She received her LPN license from UACCM in 2003. She grew up in Mountain View and now lives in Greenbrier with her husband, Chris, and dog, Chloe! She has 3 children, all grown and living on their own, and 2 precious granddaughters.
Carly davis, MS, RD, LD
Carly is a registered and licensed dietitian who lives in downtown Little Rock. She earned her Bachelor of Science degree in Nutrition and Dietetics at Harding University and completed her dietetic internship at UAMS. She completed her master's degree in Clinical Nutrition at UAMS where she engaged in research on the effectiveness of teaching kitchens in improving nutrition knowledge and confidence in medical students. Carly serves as Arkansas State Coordinator of the Vegetarian Nutrition Dietetic Practice Group. She loves creating healthful and delicious plant-based meals and educating others about the benefits a plant-focused diet can have on human health and the environment. Amanda serves as part of our administrative support team at Freeman Family Medicine and has been with us since August 2017. She is currently working on her Associate of Health Science degree from Arkansas State University. Originally from Louisiana, she now lives in Greenbrier with her husband and two children, Emersyn and Ellis.
Freda serves as our Administrator and has been with Freeman Family Medicine since 1997. She has been instrumental in turning Freeman Family Medicine into a Patient-Centered Medical Home. She earned her bachelor’s degree from the University of Central Arkansas and her master’s from The Ohio State University. She is responsible for the day-to-day running of Freeman Family Medicine. She enjoys readings, running, traveling, and seeing her grandkids in her spare time. Freda has followed a plant-based diet since March 12, 2017.
ASHLEY FYKE, RMA
Ashley serves as a medical assistant at Freeman Family Medicine and has been with us since January 2015. She received her Registered Medical Assistant license from Eastern College of Health Vocations in 2014. Ashley is largely responsible for bringing our patients back to the exam rooms and completing medication reconciliation and vital signs. She is also currently heading our involvement in the Millions Hearts Study.
Michele Joseph, BSN, RN
Michele earned her bachelors of science in nursing from the University of Central Arkansas. From there, she worked in the Neonatal Intensive Care Unit at Arkansas Children's Hospital for seven years before coming to work with Freeman Family Medicine in December of 2017. Born and raised in Mountain Home, she now lives in Conway with her husband, Brooks, and son, Weston.
Kathy has been with Freeman Family Medicine since 1997. She is our Lab Director, running our in-house laboratory and keeping us on top of regulatory requirements in the lab. She also serves as our Bone Densitometer Technician and has been doing bone densities since 1999. Kathy graduated from Hendrix with a Bachelors of Arts degree. CECILE MADDEN, CCRC, MPH
Cecile joined Freeman Family Medicine in 2015 as our Certified Clinical Research Coordinator. She brings over 16 years of clinical research experience to our clinic. She has lived here in Conway since 2009 with her husband and daughter. Cecile is a graduate of the University of Central Arkansas and the University of Arkansas for Medical Sciences, where she earned a Master’s Degree in 2007 with an emphasis in Health Policy Development and Management. Casey joined Freeman Family Medicine in 2013 as our Clinical Allergy Specialist. She is a native of Conway and earned her bachelor’s degree from the University of Central Arkansas in 2007. Londa Revis, MSW, lCsw
Londa joined Freeman Family Medicine as an in-house counselor in 2018. She received her Master of Social Work with a specialization in marriage and family therapy from the University of Nebraska and has over 25 years of experience. She also works as an in-house counselor at two other outpatient clinics in Conway. Her practice interests include cognitive behavioral therapy in the treatment of depression, anxiety, chronic pain and fatigue, grief and loss, adjustment issues, and wellness and brain heath. She and her husband, Paul, have two sons: Paul, who completed three years of service with the U.S. Army and is now in college, and Sam, who is also in college. Abbey joined Freeman Family Medicine in 2012 as our Billing and Patient Accounts Manager. She received an Associates degree from Central Baptist College and later graduated from University of Central Arkansas with a Bachelors Degree in Psychology. Originally from Western Oklahoma, she now calls Conway her home. Abbey loves fitness, cats, and swimming. She values her relationship with the Lord and tries to incorporate her favorite quote into all aspects of daily life, “Be kinder than necessary. Everyone you meet is fighting some kind of battle."
Marian SMITH, phd
Dr. Smith is a licensed clinical psychologist with a specialty in health psychology. She joined Freeman Family Medicine in a cooperative effort with Conway Counseling & Wellness Center in 2018. She earned her doctoral degree from University of Missouri-Kansas City and completed her residency in rehabilitation and neuropsychology at University of Missouri-Columbia. She provides psychotherapy for a variety of mental health difficulties including anxiety, depression, trauma, stress relationship problems, and parenting difficulties. She also works with health issues including obesity, adjustment to disability, health behavior change, and adherence to treatment recommendations. She is also a certified yoga instructor and incorporates mindfulness, relaxation, and biofeedback into her practice. Dr. Smith grew up in Florida and loves the outdoors. KAYLA SMITH, CCMA
Kayla serves as the Care Coordinator at Freeman Family Medicine. She has worked at our clinic in various roles since 2002. Originally from Leslie, she currently lives in Mayflower with her husband and daughter. As Care Coordinator, Kayla schedules all specialist appointments and ensures all consult notes are received following those appointments. She is also in charge of various other duties related to documentation of services provided and helps out in our lab. Kayla is an avid Razorback fan. WOO PIG SOOIE!!

MELISSA TYLER, BSN, RN
Melissa Tyler serves as Care Manager at Freeman Family Medicine. She has worked at the clinic in various roles since 2004. She earned her Bachelors of Science in Nursing from the University of Central Arkansas. Originally from Virginia, she now lives in Conway with her husband and four children. As Care Manager, Melissa focuses her efforts on caring for our higher risk patients who may benefit from additional services, such as weekly phone calls and additional education on their disease processes or medications. She also runs Freeman Family Medicine’s After-Hours Nurse Line.
Rebekah joined Freeman Family Medicine as part of our administrative support team in 2017. Born and raised in Conway, she is married with two small children who keep her busy with camping and outdoor activities. She received her Associates in Accounting from Kaplan University in 2010 and is currently working on her certification as a Professional Medical Coder. Rebekah has been following a plant-based diet since July 2017 and has lost 50+ pounds. In her spare time, Rebekah enjoys reading, painting, and pottery. She is an active New Life Church member.
